CONTROLLED SUBSTANCE POLICY: 

Our practice understands the medical benefit and risks of certain controlled substances. 

Due to federal and state regulations, controlled substances will only be prescribed as part of a complete treatment program. 

WE DO NOT PROVIDE ADDITIONAL PRESCRIPTIONS FOR LOST, STOLEN, OR DAMAGED MEDICATIONS.

 

Schedule II controlled substances (some medications for ADHD including but not limited to Adderall, Ritalin, Concerta, Vyvanse, and other stimulants): 

Clients must be seen once a month in order to continue treatment with a Schedule II medication. 

Previous records of past medication trials and providers must be received to our office prior to an initial prescription. 

Lab work, and/or EKG must be completed prior to the initial prescription. 

A urine drug screen will be required prior to the initial prescription and may be ordered at random times at the Prescriber's discretion. 

 

Benzodiazepines, including but not limited to Xanax, Valium, Klonopin, Ativan, Librium: 

Benzodiazepines are a class of medications that are highly addictive. Our practice policy is to prescribe benzodiazepines in VERY limited quantities and only for SHORT TERM use (less than two weeks). 

Our practice does not prescribe long-term benzodiazepines. 

Those receiving a prescription for a benzodiazepine must also be actively engaged in psychotherapy with a qualified provider. If the therapist or counselor is outside of Willow Holistic Wellness our practice requires a current release of information prior to any controlled substance prescription. 

A urine drug screen may be required prior to the initial prescription and may be ordered at random times at the Prescriber's discretion. 

 

Controlled Medications for Sleep Disorders include but are not limited to Ambien, Lunesta, and other "Z-drugs": 

Previous records of past medication trials and providers must be received to our office prior to an initial prescription. 

Those diagnosed with Obstructive Sleep Apnea must be compliant with CPAP or other treatments recommended by their pulmonologist or sleep medicine professional. 

Lab work, and/or EKG must be completed prior to the initial prescription. 

A urine drug screen will be required prior to the initial prescription and may be ordered at random times at the Prescriber's discretion. 

 

Opioid Use Disorder Treatments including Buprenorphine (Suboxone, Sublucate, Subutex):

Our practice policy is to only prescribe Buprenorphine-containing medication as part of a complete treatment program. 

We require a current release of information for all mental health, chemical dependency, and primary care providers.

Previous records of past medication trials and providers must be received to our office prior to an initial prescription. 

Lab work, and/or EKG must be completed prior to the initial prescription. 

A urine drug screen will be required prior to the initial prescription and may be ordered at random times at the Prescriber's discretion.
